This Veterinary issued booklet contains all of the information of the pet owner. This particular one was issued to a Poodle and the booklet shows age, sex color and breed as well as many other forms of information.

I will elaborate later and update information on this article shortly. The enameled tag is double sided, multi colored with 4 types of animal images silhouetted in gold on the red side. This tag corresponds with the booklet and is a combined registration of proof of rabies and the city license.
